Automated Patient Forms

We offer our patients the option of automated forms. Just click here for your online connection. Your paperwork will be ready for you when you arrive. Or if you prefer, come 15-20 minutes prior to your appointment time and fill out your paperwork in person.

Peaceful Environment

We want you to be completely at ease during your visit. When you enter Mazzola Dentistry you will be greeted by calming water running down the rock wall, cozy leather couches and chairs. Our high ceilings and earth tones offer an outdoor experience. Our restroom offers clean towels, lotion, and upscale décor.
